For characters (the heroes and the boss villain) we don't use tokens, we use dice (a six-sided for the tens digit and a ten-sided for the ones digit) and the dice show the current total hp for each character.
the same could be done with tokens. we just got tired of counting out little tokens all the time.
For non-character targets (such as minions, or environment targets) we do not have on any tokens when they are at full hp. But once they get hit (if they are still alive) then we put on tokens representing their remaining hit points. This saves us the trouble of counting out tokens for minions that get wiped out in one hit.
